
Ravenloft: The Horrors Within is arguably the most expansive dive into horror ever released as an official Dungeons & Dragons sourcebook. The tome, which releases June 16th, expands upon Ravenloft lore with new details on its Domains of Dread and the Dark Lords that rule over them. It also adds 7 horror subclasses, 2 of which are brand new to D&D 5e. But for the DM prepping to terrify their players with some unsettling battles ahead, the bestiary also offers up an impressive list of new creatures to unleash.
The Ravenloft: The Horrors Within bestiary features stat blocks for 50 horror-themed monsters. The list is a mix of returning monsters from Van Richten’s Guide, updated to the new 2024 rules, and brand-new monsters to surprise your players with. There’s nothing wrong with a good old Bodytaker Podling or Brain in a Jar, but for the purpose of this list, we’re focusing on Ravenloft monsters not previously seen in Van Richten’s Guide. So, here are 5 new, terrifying monsters, ranked from least to most likely to cause a TPK.
5) Waxwork

Challenge Rating: 3
With a CR rating of 3, this is supposedly the least formidable new monster on our list. But conceptually, it’s pretty darn creepy. The Waxwork is a medium or small construct crafted to resemble a humanoid… and determined to kill and replace said creature. These duplicates will stop at nothing to destroy the “imposter” that shares their appearance, though a rare few try to lead normal lives.
The Waxwork is a surprisingly challenging creature for its challenge rating of 3. It’s immune to many status effects and resists most types of damage, making it a formidable foe. Dungeon Masters could really create some terrifying scenarios with these monsters – imagine a party encountering deadly duplicates intent on taking their place! It’s a genuinely frightening and well-designed enemy.
4) Nightgaunt

Challenge Rating: 8
I find monsters without faces particularly unsettling, and the Nightgaunt definitely fits that description. This gargoyle-like creature flies through the night, snatching people and dropping them from a great height. It’s a powerful enemy – a CR 8 creature – but what really stays with you is the terrifying image of its head splitting open to reveal a strange, glowing light inside. That’s a truly haunting picture.
Nightgaunts are dangerous foes, especially without ways to fly or avoid falling. They’re tricky to fight because they can move around without triggering attacks from your party, and their ‘Tickle’ ability can completely disable opponents. Plus, their claws inflict poison damage – definitely not a creature you want to encounter!
3) Mordenheim’s Monster
Challenge Rating: 12
Dr. Viktra Mordenheim is one of the formidable new Dark Lords in The Horrors Within. And these monsters are her abandoned creations, constructs that have developed minds of their own. They are scary strong, with attacks taht deal somoe terrifying damage and the ability to move much more quickly than you’d think a reanimated corpse could. Oh, and they’re Frankeinstein’s monster-esque compiilation of body parts, so looking at them is scary enough on its own right.
These creatures, with a challenge rating of 12, are generally neutral and won’t necessarily attack on sight. Many are simply trying to piece together memories of their former lives. However, if provoked, they become formidable opponents with numerous immunities and resistances. Be warned: certain types of damage can actually heal them and increase their speed. This unique combination of backstory and challenging combat makes them one of the most interesting additions in The Horrors Within.
2) Elder Thing

Challenge Rating: 14
Ancient, tentacled aliens, these creatures have existed for eons, collecting knowledge and reshaping worlds. Similar to the frightening Mind Flayer, they use their tentacles to attack minds. They can lull creatures into a false sense of safety or release deadly spores that cause magical powers to backfire, harming the user. Essentially, they’re incredibly dangerous beings.
With a 14 CR, the Elder Thing is a formidable opponent. It is resistant to most damage types and immune to a handful more, with advantage on saving throws against any magical effects you throw at it. Oh, and if it manages to Charm a character, it can deal even more damage with its psychic powers. And let’s not even talk about the Eldritch Magic it wields. Of all the new monsters in this bestiary, the Elder Thing is among the most dreadful. And it’s pretty creepy to look at, too.
1) Yithian

Challenge Rating: 15
Just thinking about the Yithians – with their worm-like bodies and terrifying faces – gives me the creeps. Like their ancient enemies, the Elder Things, they roam the universe collecting knowledge. But their method is truly disturbing: they casually swap minds with other beings, sometimes trapping the original consciousness inside their own monstrous form. Definitely not something I’d want to experience!
The Yithian is a formidable foe, boasting a Challenge Rating of 15 – one of the highest in The Horrors Within (excluding the Dark Lords themselves). It’s incredibly resistant to magic, gaining advantage on saving throws against spells and magical effects, and its thoughts are impossible to decipher. To top it off, this worm-like creature has dangerous pincers alongside its head, allowing for devastating multi-attacks combined with psychic damage and eldritch magic. Perhaps most frighteningly, the Yithian can swap minds with others, giving the Dungeon Master a truly terrifying way to pit the players against each other with potentially disastrous consequences.
