Before Robert Downey Jr.’s anticipated comeback as Doctor Doom in Avengers: Doomsday, he previously portrayed another villain on television screens, appearing in a renowned episode of Family Guy as the Fat Guy Strangler. Over its 25-year run, Family Guy has welcomed numerous big-name stars for guest roles, but Robert Downey Jr.’s performance might be the most unforgettable, given his unusual connection to the extended Griffin family.

In the Season 4 episode of “Family Guy” titled “The Fat Guy Strangler,” Lois Griffin uncovers a long-hidden secret: she has an older brother who had been institutionalized due to mental health issues, a fact her parents never shared with her. Upon learning this, Lois rushes to rescue him. Meanwhile, the character Patrick Pewterschmidt makes his debut in both the Griffin household and the show itself. Portrayed by Robert Downey Jr., it’s later revealed that Patrick has been confined due to a valid reason – he has a habit of strangling overweight men after being triggered by Peter, which sets off a series of chaotic events.

Peter discovers his own weight issue and forms a group with other heavyset men in town, which they call the “National Association for Plus-Sized People.” Meanwhile, Lois’ brother Patrick (voiced by Robert Downey Jr.) makes an appearance for the first time. Initially, he appears peculiar as he seems to live with an unseen spouse. However, Lois chooses to trust that he means no harm. This belief is tested when Patrick embarks on a murder spree, targeting overweight men by strangling them.

Known as the “Fat Man Strangler,” it’s disclosed that Patrick harbors a grudge against overweight men, stemming from his childhood trauma of witnessing his mother having an affair with Jackie Gleason. This animosity reaches a boiling point when Peter mimics Gleason in front of Patrick, nearly causing Patrick to choke Peter to death. Fortunately, Lois intervenes before it’s too late, and Patrick is returned to the mental institution. However, he does reappear in the series later on.

Does Patrick Ever Come Back?

Since Patrick belongs to the larger Griffin Family clan, you can expect him to make further appearances in the series. Family Guy reintroduces the character in Season 10’s episode “Killer Queen,” where he is once more suspected as a killer when a new overweight strangler emerges. However, it’s revealed that he’s not guilty and plays a significant part in apprehending this new villain before Chris meets harm. In this subsequent appearance, a different voice actor, Oliver Vaquer, portrays the character.

Since the airing of the “Family Guy” episode in 2005, Robert Downey Jr. has been involved in nothing but major productions. Following a decade-long stint playing Tony Stark in the Marvel Cinematic Universe, he is now set to reprise his role as Doctor Doom in the upcoming “Avengers: Doomsday,” which is planned for release in 2026. It’s likely that many other actors will also make a comeback for this new “Avengers” film, but at the moment, Downey Jr. remains the only confirmed star for the project as of now.
