To succeed in the tougher parts of Ninja Gaiden 4, you’ll need to maximize your health. Improving this stat requires finding upgrades hidden throughout the game, exploring off-the-beaten-path areas, completing optional challenges, and progressing through New Game Plus.

Chapter 1
Phantasmal Herb #1
As a reward for completion of Mission 2.
Chapter 2
Black Scale #1
Go left from the monorail track after the battle with the drones and flying enemies.
Once you get this, it will automatically combine with the Phantasmal Herb you already have, creating your first Dragon Lord’s Grace.
Chapter 3
Black Scale #2
As a reward for completion of Mission 4.
Phantasmal Herb #2
In a chest at the top of a stairway, after the Burabura Lantern Daemon fight.
Chapter 4
Black Scale #3
You’ll find a chest on top of a wall – you’ll need to run and jump to get to it. To find it, take the left path when the road splits after the initial sneaking section.
Black Scale #4
As a reward for completion of Mission 8.
Chapter 5
Phantasmal Herb #3
As a reward for completion of Mission 11.
Phantasmal Herb #4
As a reward for completion of Mission 12.
Chapter 6
Black Scale #5
Once you cross the rope bridge, turn around completely instead of continuing forward. You’ll find what you’re looking for in a chest to your left.
Chapter 8
Black Scale #6
As a reward for completion of Mission 15.
Chapter 9
Black Scale #7
When the wave sequence starts, head left immediately after going through the glowing gate. Stay on the left side until you get to the fighting area with the two chests – the Black Scale is inside the chest on the right.
Phantasmal Herb #5
As a reward for completion of Mission 17.
Chapter 11
Phantasmal Herb #6
In one of four chests found at a station, post-completion of Mission 19.
Chapter 12
Phantasmal Herb #7
You’ll find a chest slightly to the right, at the top of the small set of stairs, before you enter the area with the red lasers.
Black Scale #8
As a reward for completion of Mission 22.
Chapter 19
Phantasmal Herb #8
In a chest to the left of the shaft you will ascend, post-battle versus Ryu.
This is the final health upgrade available in a standard New Game playthrough. Two more health upgrades are available, but you can only get them by starting a New Game+ with a completed save file and taking on the new missions that unlock.

As a reward for completion of Mission 26.
