In a surprising turn for the ITV drama Emmerdale, Cain Dingle, played by Jeff Hordley, revealed he has been diagnosed with prostate cancer.
Things couldn’t be more difficult for the leader of the Dales family right now. His wife, Moira (Natalie J Robb), is in jail, falsely accused of killing Celia Daniels (Jaye Griffiths) and Anya Berisha (Alia Al-Shabibi).
While Sam and his sister-in-law Lydia were taking their children to school, Moira called Sam to wish him well before his appointment. The show used a unique technique, letting the audience hear Cain’s thoughts as he worried about the day.

At the hospital, another patient unfortunately made him feel even more anxious. The man explained that he’d been treated before for prostate cancer, and it had now spread.
During my check-up, the doctor gave me some really tough news. He explained I had a pretty aggressive cancer, but thankfully it was localized, and we needed to start dealing with it right away. It was a shock, of course, but knowing it hadn’t spread gave me a little hope.
Honestly, the scene hit me hard. The doctor calmly explained he’d need a pretty serious operation – a radical prostatectomy – and laid out the potential side effects. It wasn’t sugarcoated at all. She made it clear there was a real chance he could struggle with incontinence and, even more devastatingly, experience erectile dysfunction. It was a brutally honest moment, and really drove home the weight of his diagnosis.

The doctor asked about Moira’s whereabouts and suggested they talk more about it later if he was willing. Feeling upset, Cain quickly left and went back to the village.
I have to say, seeing Al Chapman around the village was… unsettling. Several people ran into him – Ross Barton among them – but it was his interaction with Sarah Sugden that really stood out. The poor girl noticed immediately that ‘Grumpy’ – as she calls him – wasn’t himself, and he reacted with some surprising anger. Clearly something’s eating at him, because he practically bolted to The Woolpack for a drink afterwards. It’s setting up something big, I can feel it.
He wished he’d gotten a PSA test sooner, as it could have detected the problem earlier.

Cain’s quiet time was interrupted twice: first by Sam, then by Dr. Cavanagh. Both assumed he was upset about Moira being in prison, but Cain told them to give him space.
I found a really poignant scene early on where the main character visits his dad’s grave. It’s there he’s joined by Sarah, and you instantly get the sense she’s the one person in his life who gets it. We quickly learn why – Sarah is battling Fanconi anemia, a rare and devastating genetic condition. It causes serious health problems like bone marrow failure and leukemia, and unfortunately, it significantly shortens her life expectancy. It adds a powerful layer to their connection, knowing what she’s facing.
He hesitated at first, but finally shared everything. She reassured him she would support him and that he wouldn’t have to go through it by himself.
But when will he divulge his diagnosis to Moira?
