Shonda Rhimes will always remember these shocking visits at Grey Sloan Memorial Hospital.
The creator of Grey’s Anatomy has spoken about her choices to remove popular characters from the show, and how she personally deals with the sadness those decisions create.
Shonda Rhimes recently reflected on some character deaths in her shows, saying she sometimes wonders how she could have done them. However, she explained on the March 10th episode of Craig David’s Glass Half Full podcast that, at the time, the story felt like it demanded those choices and she didn’t see any other way to go.
The 56-year-old clarified that she didn’t develop the popular ABC show—featuring stars like Ellen Pompeo, Chandra Wilson, Kate Walsh, and Jesse Williams—simply to satisfy the audience.
She explained that her primary focus is serving the story itself. It’s not about pleasing fans or maintaining friendships with the actors involved. She has to make decisions based on what’s best for the narrative, even when it’s difficult. She believes this is crucial because prioritizing anything else would prevent her from telling the most compelling story possible.
As a longtime fan, I’ve definitely shed my share of tears watching Grey’s Anatomy over the years. It’s been a rollercoaster, and sadly, that includes some truly shocking deaths. I still remember being devastated when Mark Sloan, played by Eric Dane, tragically died from his injuries after the plane crash in season nine. And the losses of George O’Malley, portrayed by T.R. Knight, and Derek Shepherd, played by Patrick Dempsey, were just heartbreaking – both of them dying in separate, awful bus accidents. It’s hard to believe it’s been two decades of this show and all the emotional moments!
However, there’s one demise from the show that still haunts Shonda—both as a writer and a viewer.
Shonda Rhimes said the moment they killed off Denny, played by Jeffrey Dean Morgan, really stood out to her. She admitted she followed the story’s needs, even though she disliked putting Izzie Stevens, played by Katherine Heigl, through that heartbreak in the earlier seasons. Ultimately, she felt it was a necessary part of the narrative.

This isn’t the first time the former Scandal star has talked about her tendency to eliminate characters to make her show’s story more complex. She’s also made it clear that these difficult decisions aren’t meant to be personal against the actors involved.
When a character needs to be written off a show, it’s always a story decision, not a personal one. I prioritize what’s best for the narrative, regardless of how much I like the actor or their performance,” she explained to Entertainment Weekly last May.
She explained it plainly: artists create, and audiences are free to share their thoughts and reactions – that’s just part of the process.
Keep reading to look back at the most tragic deaths in Grey’s Anatomy’s history…

Little Mousey, who often felt overlooked and strange, tragically died trying to help Richard. She found him unconscious on the ground and, stepping into the same water puddle, was electrocuted. It was a well-intentioned but fatal error. This loss deeply affected Shane, who blamed himself and, exhausted from lack of sleep, made a critical mistake during surgery, resulting in the death of Alex’s father. It was a terrible chain of events.
Tear count: Sorry Heather, our eyes were dry.

Okay, let’s talk about Reed. Honestly, she wasn’t the most beloved character – many considered her the least sympathetic of the Mercy West crew. But her death? Totally shocking. One minute she’s there, the next Gary Clark, still reeling from loss, pulls a gun out of nowhere and… well, it happened. That single gunshot kicked off the entire hospital shooting storyline, and honestly, it left a lasting impact on all of us viewers. It was a moment we definitely didn’t see coming and it really changed the show’s trajectory.
Okay, so when Reed… well, let’s just say it didn’t exactly break my heart. Honestly, I barely reacted at the moment! I was just completely stunned, like my brain short-circuited. I think I was too busy reeling from everything else to even feel sad about it, if that makes sense. It wasn’t a tearful moment, not even close!

That prolonged goodbye was incredibly heartbreaking, and watching George suffer through it was truly painful. It led to his rushed decision to marry Callie, a storyline many fans still wish they could forget. However, it did finally give us the deeply touching connection between George and Cristina that we’d been waiting for.
Tear count: Several for poor George.

Ellis had been declining for a long time due to Alzheimer’s, and Meredith had essentially lost her mother years before her actual death. When Ellis finally passed away, she offered Meredith the peace she desperately needed, telling her, “You are anything but ordinary.”
Okay, let’s talk about emotional moments! There was just one tear during this scene, and honestly, it was a beautiful release. It wasn’t sadness, though – it was pure relief that Meredith finally got the heartfelt goodbye with her mother that she – and we, as the audience – had been hoping for. It was a single tear of closure, and it really landed.

Pink mist. Never forget.
We weren’t so much upset about what happened to the Bomb Squad guy as we were just emotionally overwhelmed by the stress of the episode.

April and Jackson’s baby was born early because the child had a rare and severe bone disease called Type 2 osteogenesis imperfecta, and sadly, only lived for a few minutes. This was a heartbreaking moment on television, but April and Jackson showed incredible strength while facing such a difficult and painful decision.
Tear count: So, so many.

This patient’s death in the hospital shooting was particularly tragic because it could have been prevented. Despite being seriously injured, he might have survived if Dr. Bailey could have gotten him to surgery. When she discovered the SWAT team had cut the power to the elevators, she knew she couldn’t reach him in time. Though devastated, Bailey remained composed, focusing on ensuring the patient’s final moments were as comfortable as possible.
At first, Bailey only shed a few tears. But when he had to tell Charles he was going to die, despite assuring him everything would be alright, he broke down completely and began to sob uncontrollably.

Even though the chief had grieved the loss of his wife long before she passed away – she had Alzheimer’s and had developed feelings for someone else at the nursing home – her death was still incredibly painful. Bailey put her own wedding on hold to try and save Adele’s life, but sadly Adele died that same day. Despite everything, Richard still came to Bailey’s wedding to be there for his friend, which was a truly selfless act.
Meredith started crying—lots of tears—when Richard shook his head to tell her the sad news that Adele had passed away.

While Henry initially married Teddy for practical reasons, they genuinely fell in love, which made his sudden illness even more devastating. They were just starting their life together when he required urgent surgery, and tragically, Cristina was unable to save him – unaware she was operating on Teddy’s husband. The reveal that the patient she lost was Teddy’s husband sent shockwaves through everyone. To make matters worse, Owen delayed telling Teddy because she was in surgery herself. It was a truly painful situation.
Tear count: Hiccup-ugly-crying.

It was heartbreaking for Lexie. After longing for Mark Sloan for so long, she finally found happiness with him – but only moments before a plane crash left her critically injured. As she lay dying, coughing up blood, she and Mark shared their love and dreamed of a future they knew they’d never have.
Tear count: steady streams of water running down our cheeks.

Mark Sloan, affectionately known as ‘McSteamy’ from Grey’s Anatomy, is remembered for one of the show’s most iconic scenes. His death in the plane crash was particularly heartbreaking. Not only did he lose Lexie, the woman he loved, in a devastating way, but he also endured the agonizing realization that he was fatally injured and faced saying goodbye to his friends and daughter before passing away. He will be deeply missed.
Tear count: All these years later, we’re still crying.

We’ll always remember the shock of realizing Meredith discovered John Doe was actually 007 – or, as we knew him, George O’Malley. That reveal was so intense, it felt like a first heartbreak! While his departure from the show was incredibly sad – seriously, that bus scene was awful – he died a hero, selflessly jumping in front of the bus to save a stranger.
We cried so hard – big, messy sobs that left us a wreck. Just remembering it makes us want to cry all over again.

Derek Shepherd’s death on Grey’s Anatomy was incredibly shocking, but many fans feel it didn’t have the emotional impact it deserved. Because his death and the aftermath happened so quickly – within a single week and then fast-forwarded a year – it felt rushed and didn’t allow viewers to fully grieve. It was so sudden, and the show moved on so rapidly that it still doesn’t quite feel real to some – is Derek really gone?
Tear count: Total dehydration, since we cried all the water in our bodies out.

The death of DeLuca was incredibly impactful for so many reasons. It unfolded across both Station 19 and Grey’s Anatomy, and it had been a long time since a major character had died on either show. It happened right after he finally caught the person who had kidnapped the child he’d been relentlessly pursuing, following a difficult struggle with his mental health. It genuinely felt like he was going to be okay at one point. We also got to see him share a poignant goodbye with Meredith during her COVID-19 coma and then have a peaceful reunion with his mother as he died. And unlike Derek’s death, this one wasn’t leaked to the press beforehand, so it completely caught us off guard. We were heartbroken and cried for days – even while talking to Giacomo Gianniotti on the phone. Weeks later, the memory of it still brings tears to our eyes. We’re still reeling from the loss.
Tear count: Gross, messy, embarrassing river of sobs.

Denny’s death was absolutely devastating. After battling so many health issues – including surviving a faulty LVAD and receiving a heart transplant – he tragically died of a stroke, alone in his hospital room, right after Izzie said yes to his proposal. It’s considered the most heartbreaking death in the show’s history, and rightfully so, given how much the story built up to it and the impact it had. It happened early in the series, when the show was at its best, and it remains one of the most iconic TV deaths ever. The image of Izzie in her pink gown, collapsing into Alex’s arms over Denny’s body, is unforgettable.
Tear count: Niagara Falls has nothin’ on us!
