
While several books have explored the world of Ravenloft for Dungeons & Dragons 5th Edition, Ravenloft: The Horrors Within is the first major update under the latest rules. It also represents the most comprehensive exploration of horror the game has seen. This sourcebook details 16 distinct Domains of Dread, building on familiar locations from Van Richten’s Guide to Ravenloft and introducing three completely new and terrifying realms for players and Dungeon Masters.
The book The Horrors Within expands on the Domains of Dread, many of which—like Barovia—will be recognizable to Dungeons & Dragons players. However, it introduces two completely new Domains, and elevates a third from a minor setting to a fully developed realm. If you’re most interested in fresh content, these new Domains are the highlight. I’ve had a preview and will detail the horrors within, starting with the least frightening and moving towards the most terrifying.
3) Sithicus

Sithicus has a dark and fantastical atmosphere, linked to the Padalin Tarokka card because it was once ruled by a powerful, dark knight. It’s a twisted, broken version of the world of Krynn, filled with the remnants of a lost golden age. This elven realm was once beautiful, but now it’s covered in ruins and scarred by a devastating fire. It’s definitely not a relaxing place to visit, though it isn’t quite as terrifying as some of the other regions described in The Horrors Within.
Sithicus, a land of ruins and eerie violet flames, is dominated by the Great Chasm – a massive, bottomless pit that few dare enter and even fewer escape. While the Chasm itself is truly frightening, and enough to mark Sithicus as one of the Domains of Dread, its ruler, Lord Soth, is surprisingly more melancholic than actively evil. He’ll defend his territory if challenged, but doesn’t seem particularly enthusiastic about being a Dark Lord. This makes Sithicus perhaps the least outwardly terrifying of the new Domains, though it still offers plenty of potential for a genuinely frightening campaign.
2) The Shadowlands

The Shadowlands were briefly mentioned in the ‘Other Domains of Dread’ section of Van Richten’s Guide to Ravenloft, so some players and Dungeon Masters may already be familiar with the concept. However, The Horrors Within significantly expands on that initial idea, turning it into a complete Domain of Dread with its own Dark Lord. This makes Shadowlands one of the new horror settings available. The Domain draws heavily from Gothic horror and is associated with the Torturer card in the Tarokka deck. Its new Darklord, Ebonbane, is among the most terrifying villains in the book.
The Shadowlands are a dangerous place haunted by cursed knights, forced to endlessly fight recurring enemies. They also face a terrible threat from Ebonbane, a sentient sword determined to destroy them. Getting lost is another danger, as the Phantasmal Forest is a constantly changing, dreamlike maze. It’s a classic, spooky Gothic horror environment, made even more unique by a sword that can corrupt and control the undead. While it’s a strong horror setting, it doesn’t quite reach the same level of dread as the most memorable new area in The Horrors Within.
1) Innsmouth

Innsmouth is famously the home of Cthulhu, the terrifying entity who now rules this new Domain. This is the first time Cthulhu has officially appeared in D&D, and he’s brought with him a truly dreadful place to govern. Linked to the Bishop Tarokka card, this Domain features a bizarre coastline filled with unsettling formations – from odd statues to eerie, misty pillars – creating a landscape that feels completely foreign. To make matters worse, it’s overrun with cults all fighting for power.
Innsmouth is a terrifying place, not just because of its creepy atmosphere and ruins, but because it holds Cthulhu captive. Even chained, Cthulhu’s influence spreads throughout the town, inflicting disturbing visions on anyone who visits. Its plan is to lure someone into releasing it, which would mean the destruction of everything. It’s genuinely frightening, making Innsmouth the most chilling of our new Domains.
