
John C. McGinley is back as Dr. Perry Cox in the eighth episode of the Scrubs revival, titled “My Odds.” While he appeared at the start of the new season, scheduling issues have kept both Dr. Cox and Carla Espinosa (played by Judy Reyes) largely absent. His return is a pleasant surprise, but the episode quickly takes a surprisingly emotional direction. Spoiler alert: The following contains spoilers.
Dr. Cox’s return to Sacred Heart isn’t simply to make J.D.’s life difficult; he’s also dealing with a health crisis of his own. He collapses and is diagnosed with microscopic polyangiitis, a rare autoimmune disease with potentially serious complications. This leads to a deeply emotional scene between Cox and J.D. that fundamentally changes their dynamic. As actor John McGinley explained to TV Line, that was the core intention behind the storyline.
Zachy suggested a storyline where Cox returns to the show because of a medical issue. He explained that the story would flip the usual mentor-mentee dynamic, with Cox now needing help. I thought that was a fantastic idea, offering a lot of potential for the writers. Aseem Batra, the new executive producer, really delivered with episode 108 – it’s as strong as the classic Brendan Fraser episode and one of the most ambitious half-hours we’ve created.
The Dr. Cox Twist Is Sad, But Perfect For The Scrubs Revival

As a huge Scrubs fan, I really appreciate that the revival hasn’t been afraid to take risks. The show could have just rehashed old jokes and moments, but it hasn’t. Things like J.D. and Elliot being divorced, and especially Dr. Cox’s illness, feel like natural, and honestly, brave steps forward for these characters we love. It really shows how much time has passed since the original series, and as John McGinley said, it lets us see a side of Cox we’ve never seen before – a much more vulnerable side.
We caught a quick glimpse of it at Ben’s funeral, but everything felt muted and unreal, like a silent movie. Zachy simply asked, ‘Where are we?’ and then the same quietness fell over Cox. It’s a thoughtful look at death and what it means to be human – dealing with fear, feeling unprepared, and trying to make peace with things. It felt like a scene from a movie, and I needed some space.
Now that J.D. is the new Chief of Medicine at Sacred Heart, this storyline makes perfect sense for his character. He’s always sought Dr. Cox’s approval and relied on him as a backup, but now that support is gone. Not only has that safety net disappeared, but their roles are reversed, forcing J.D. to truly prove himself. According to show creator Bill Lawrence, Dr. Cox’s medical condition was created to bring him back into the story for seasons 2 and 11 and to create more tension and challenges for J.D.
The writers wanted to give Cox a health issue that would keep him around next season, but still create dramatic tension – something he’d need Zach to help with, or at least try to fix. They’re hoping to include Cox more in Season 2 than they could in Season 1 because the filming schedules for Rooster and Scrubs should be less conflicting if the show gets renewed, and that’s really exciting for them.
I believe J.D. needs to face higher stakes and more serious consequences in the second season. Right now, the danger mostly falls on those who work for him, which isn’t directly affecting J.D. himself. Characters like Cox, Kelso, and the Janitor are all strong threats to J.D., and I’m hoping Cox will return as a significant challenge or source of conflict for him. I’m just speculating as a viewer, but that’s the kind of drama I’d like to see.
Whether Scrubs will continue isn’t certain, but fans will be happy to hear that John C. McGinley (Dr. Cox) is expected to have a bigger part if it does, though it took a health scare for that to happen. At least Dr. Cox will be in the Season 10 finale, alongside other beloved characters like Jordan Sullivan (Christa Miller) and The Janitor (Neill Flynn). His illness, and how it affects him, J.D., and everyone else, will likely be central to the finale, and hopefully to the show going forward.
The Scrubs Season 10 finale will release on Wednesday, April 15th on Hulu.
