Looking to earn all the trophies in Silent Hill f by collecting every document? This guide will show you where to find all 36, but be aware – it’s a challenging undertaking!
Achievements (Trophies)
The latest Silent Hill game includes more than 60 documents to find, but you only need 36 of them to unlock certain achievements.
- Good Intentions
- Fires of Jealousy
- Fate of the Apostate
- A Mike Possessed
- The Truth about the Tsuneki Family
You’ll need to play the game at least three times to unlock all five achievements. The other files are just extra story details and aren’t needed to finish the game.
Check out each document that counts and where to find them:
First Playthrough
Corridors
Document #1 – Housemaid’s Note (1)
This first document is located in one of the library’s nooks on the floor.
Document #2 – Family Physician’s Log (1)
Return to the hallway and enter the room immediately to the left of the previous one. You’ll find it on a table inside.
Document #3 – Family Physician’s Log (2)
Go back down the hall a bit, and on your right, you will see this document lying on the floor.
Worship Hall
Document #4 – Sakuko’s Diary (1)
You can find this document in the far eastern section of the Worship Hall. To reach it, first enter the Hokora Shrine and pull the lever. This will unlock a path through the blue doors. Once inside, flip the second lever to open the white door and access the document.
You’ll find the document through the white door, on the table against the north wall.
Document #5 – Sakuko’s Diary (2)
Continue heading east, and then travel as far north as you can. To reach this area, you’ll need to activate a few more levers. Start by going to the locker room and flipping the lever there. This will unlock the green door, allowing you to proceed.
Okay, so after squeezing through this really small opening, you’ll find the room right on your right. It’s just sitting there on a shelf, easy to spot once you’re through!
Document #6 – Sakuko’s Diary (3)
Head south to the room containing the second Hokora Shrine. You’ll find it in a corner, resting on a small table.
Main Hall
Document #7 – Family Physician’s Log (3)
You have to open the first door, and then you will find this document on a desk to your left.
Document #8 – Housemaid’s Note (2)
To get Housemaid’s Note (2), start by completing the first ritual. Then, go to the first floor and move to the next room. Open the metal door, and at the far end of that room, you’ll find the document on a table. There’s also an Omamori (Crow) item right next to it.
Document #9 – Family Physician’s Log (4)
To obtain this document, you need to finish the third ritual. This ritual lets you find secret rooms by pressing and holding the L2 button. You’ll know you’ve found one of these hidden areas because they’ll be marked with blood and fox imagery.
Before you pass through the metal door, look to your left – there’s a hidden room you can access. Inside, you’ll find the Family Physician Log (4) sitting on a desk.
Ebisugaoka Fifth Visit
Document #10 – Clinical Trial (1)
Keep walking through the open area in the mountains. You’ll encounter an enemy that appears from a mass of flesh. Once you’ve dealt with it, head to the right – you’ll find a yard beside a house. The Clinical Trial document is on a crate in the yard, next to a burlap sack.
Document #11 – Strict Mother’s Letter (1)
You’ll soon come to a fork in the road. Take the left path and continue until you see an Inari Shrine. The document you need will be right in front of the shrine, on the ground.
Document #12 – Strict Mother’s Letter (2)
Enter through the wooden gate and walk straight ahead until you reach the porch. You’ll find the document sitting on the porch floor.
Document #13 – Strict Mother’s Letter (3)
The document is located right by the fence around the house. You’ll find it just around the corner from the front door, next to a small Hokora Shrine.
Treasure Hall
Document #14 – Family Physician’s Log (5)
Begin in the main room, facing the large painting. Turn right to find a door and open it. Once through the door, you’ll see the Family Physician’s Log (5) directly in front of you, resting on a raised platform.
Document #15 – Housemaid’s Note (3)
Walk to the end of the hallway until you see a room with a red platform and candles. Watch out – there’s an enemy to your right before you enter. Once you’re past the enemy, turn left, and you’ll find the document on a table at the back of the room.
Okay, so I just found a collectible! It’s right there on the red platform with all the candles. When I opened the box, I got a Black Sparrow Crest, which is awesome!
Document #16 – Housemaid’s Note (4)
Leave the two rooms you’re currently in and head to the room where you previously saw the monster. If you’re still in the red platform room, it’s on your left. You’ll notice a hidden wall there – open it to reveal a passage, then go through the double doors on the right.
You’ll now return to the main room. Head for the doors on the left, and you’ll find the document at the far end on a wooden platform.
Document #17 – Strict Mother’s Letter (4)
Please go to the door on your right. When you open it, you’ll find the document on the floor, right in front of you on the mat.
Document #18 – Strict Mother’s Letter (5)
After you retrieve the last document, proceed straight through the doors. You’ll see a large enemy marking the way to your next destination.
After defeating the enemies, you’ll find a red altar with a black box. Opening the box will reward you with a White Sparrow Crest. To get back to the main room, go through the doors to the left of where you found the box.
Walk straight into the next room. You’ll notice animal statues and bloodstains on the floor. Head to the right side, and you’ll find the document on a row of narrow tables against the far wall.
If you’ve collected all the documents – the Family Physician Logs, Strick Mother’s Letters, and the Housemaid’s Notes – you’ll now be able to uncover the truth about the Tsuneki Family.
Second Playthrough
After you finish the game once, you’ll unlock the Brooch, which lets you open locked Doors and Inari Boxes. This will reveal extra documents and information you couldn’t find during your first time playing.

Ebisugaoka First Visit
Document #19 – Diary of Revenge (1)
You’ll find this item after visiting the first Hokora Shrine. Head down the stairs and turn right into the small alcove. On the ground, you’ll see a box containing the Diary of Revenge (1).
Ebisugaoka Second Visit
Document #20 – Diary of Revenge (2)
On the east side of your map, you’ll find a Hokora Shrine along a main road. Just past the shrine, turn left into a small garden. You’ll find a box there that contains the document you’re looking for.
Ebisugaoka Middle School
Document #21 – Diary of Revenge (3)
Okay, so if you’re standing outside Ebisugaoka Middle School, you’ll want to head towards the far western side. There’s a set of stairs on your right – take those, then turn left and follow the outer wall. Keep going until you see a window you can climb through. Once you’re in, look for a box just a little bit to your northeast, and that’s where you’ll find Diary of Revenge (3)!
Worship Hall
Document #22 – Sakuko’s Diary (4)
You can find this document in the first room of the Worship Hall’s eastern side, the one with the Hokora Shrine. It’s located on a shelf in the corner.
Document #23 – Sakuko’s Diary (5)
Sakuko’s Diary (5) is pretty straightforward. It is on a bench within the locker room.
Document #24 – Sakuko’s Diary (6)
Continue to the east wing and find the room with the third Hokora Shrine. The document you need is on a table in the corner of that room.
Ebisugaoka Fourth Visit
Document #25 – Diary of Revenge (4)
Climb the stairs until you reach the gazebo. Then, walk to its left side, and you’ll find the box next to some stones.
Ebisugaoka Fifth Visit
Document #26 – Clinical Trial (2)
You’ll have to battle someone first. Once that’s over, keep heading downhill until you spot a house with stairs on your left. Climb those stairs to the very top, and you’ll find the document right beside the fifth Jizo statue.
Document #27 – Diary of Revenge (5)
After you walk through the park, you’ll find the house you saw earlier in the game. It’s located just before a Hokora Shrine, on the left side of the yard.
Shimizu Residence
Document #28 – Diary of Revenge (6)
You will have to navigate through the house a bit before getting this document.
Okay, so I started in the first area and headed west down this hallway. Then I came to a door and kept going straight, past two rooms. The box I was looking for was kinda hidden – it was just sitting on top of a bunch of other boxes and blended right in, honestly. It took me a minute to spot it!
If you’ve already found all the other pages from the Diary of Revenge, you’ll now unlock the Fate of the Apostate achievement.
Third Playthrough
Similar to what you experienced on your second time playing, certain items are only available if you play the game more than once. The next eight items fall into that category – you’ll need multiple playthroughs to get them.
Worship Hall
Document #29 – Rinko’s Diary (1)
You can find Rinko’s Diary (1) in her room, on a shelf below a doll, located on the northwest side of the Worship Hall.
Document #30 – Rinko’s Diary (2)
Continue heading west through the west area until you reach the Classroom. You’ll find the second entry to Rinko’s Diary on one of the student desks inside.
Document #31 – Rinko’s Diary (3)
Remain in the classroom and walk to the front, where you’ll see a flickering desk lamp. Touching the lamp will make the room glow red. Then, look along the left wall – you’ll find the document on a set of shelves.
Document #32 – Rinko’s Diary (4)
Okay, so I’m still in the classroom, and after messing around with the lamp, I found this document hiding on the shelves in the back. It was pretty easy to miss!
Document #33 – Rinko’s Diary (5)
Head south to the Chizuruya General Store. Inside, you’ll find a desk lamp – interacting with it will change the room’s color to red. The document you’re looking for is on the right wall.
If you’ve found all five of Rinko’s Diary entries, you should now unlock the “Fires of Jealousy” trophy.
Document #34 – Sakuko’s Diary (7)
Going to the locker room in the east area, you will find this document on a bench.
Document #35 – Sakuko’s Diary (8)
From the east side of the Worship Hall, head south until you reach a shrine room – it’s close to the Hokora Shrine. You’ll find the document in the corner of the room, on a small table.
If you found all eight of Sakuko’s Diary documents, you should now have unlocked the achievement, “A Miko Possessed”.
Ebisugaoka Fifth Visit
Document #36 – Clinical Trial (3)
Once you’ve been to Hokora Shrine, continue along the path until you see a house on your left. The document is outside on a long table just to the left of the house.
This will unlock the Good Intentions trophy if you have collected all three Clinical Trial documents.
You’ll need to gather thirty-six documents for trophy hunting. It’s a challenging process, and only the most determined hunters will complete it.
