
The upcoming revival of Scrubs is bringing back most of the original cast, but getting everyone involved has been a bit complicated. While Janitor (Neil Flynn) and Dr. Kelso (Ken Jenkins) haven’t been confirmed to return yet, it’s likely they will appear eventually. It seems the writers intentionally made things harder for themselves by choosing to bring back a character who presents a bigger writing challenge, rather than simply reuniting with those two familiar faces.
Rob Maschio famously played Todd on all nine seasons of Scrubs, which originally aired from 2001 to 2010. While Maschio wasn’t a traditional actor, he became a beloved fan favorite known for his quick, funny lines. However, Todd’s often inappropriate jokes wouldn’t be well-received by audiences today. While Scrubs always had a lighthearted approach to reality, bringing Todd back exactly as he was might have gone too far. Luckily, the show’s revival has cleverly found a way to include the character by creating a completely new persona for him.

After a series of short previews, the trailer for the Scrubs revival is finally here! It’s fantastic to see the original cast back, and exciting to get a glimpse of how the new characters will fit into the familiar setting of Sacred Heart, which hasn’t been seen in over 15 years. A standout addition to the cast is Vanessa Bayer, known from Saturday Night Live. She’ll be playing Sibby, who leads Sacred Heart’s wellness program and is responsible for overseeing the behavior of everyone at the hospital.
I’m already fascinated by the dynamic between Sibby and Dr. Cox! The trailer showed a glimpse of their interactions, and it’s hilarious. Dr. Cox is still his wonderfully abrasive self – it’s like his teaching style hasn’t evolved at all. He’s laying into one of the new doctors, and Sibby just appears and tells him to tone it down. When Cox asks if he has ‘three strikes’ left, Sibby deadpans, ‘You have 900 strikes!’ It perfectly captures their relationship: Cox knows the updated rules, he just doesn’t care about following them. It’s classic Cox, honestly, and I can’t wait to see more of their banter.
With growing awareness of mental health, particularly at work, bringing back the character Sibby for the new Scrubs series makes perfect sense. It allows Dr. Cox to remain his typically blunt self while also recognizing that some of his behavior wouldn’t be acceptable today. While a real-life doctor with his track record would likely face serious consequences, the show’s comedic nature allows for more flexibility. Sibby seems helpful in addressing Dr. Cox’s tough methods, but she’ll be even more important to the success of the Scrubs revival.
Todd, from Scrubs, was always the character most likely to say something shocking. While his enthusiasm for high-fives still holds up, his habit of turning everything into a crude or sexual joke is seen as more problematic now. The network, ABC, is likely to try and limit these inappropriate outbursts, and Sibby seems poised to step in. Much like she challenged Cox’s harsh treatment of the staff, she’ll probably be quick to intervene whenever Todd is about to say something offensive – which is most of the time.

It’s clear Robert Maschio really enjoyed playing the Todd. He’s often talked about his time on the show since it ended in 2010, particularly in his many Cameo videos. He’s also expressed a lot of excitement about returning for the revival, which makes sense considering how popular his character was. Seeing him wear the flaming surgical cap again feels right! If Sibby is going to be monitoring the Todd – and I suspect she will – then bringing her back is a brilliant move. However, it does make you wonder just how much of a role Maschio will have in the new season of Scrubs.

In January 2025, Zach Braff (Maschio) hinted he was also thinking about the revival. He shared on Facebook how much he’d recovered from the difficult time during the COVID-19 pandemic and was excited to return to Scrubs. However, he added that he wasn’t sure how much he’d actually be in the new season. I was puzzled at the time – why bring him back if he wouldn’t have a significant role? Now, I think I understand the reasoning.
I expect Sibby will consistently be nearby Todd, staying out of view until she needs to intervene. When she anticipates Todd saying something inappropriate, she’ll interrupt him. Considering how much of Todd’s original dialogue was offensive or sexually suggestive, it’s likely Sibby will censor most of his lines. This could explain why the actor, Maschio, worried about how much he’d appear in the final cut of the new Scrubs episodes – perhaps there simply wasn’t much usable footage of him.
