Why Dr. Cox Chose JD Over Elliot in the ‘Scrubs’ Revival

Be warned, spoilers ahead for the first two episodes of the Scrubs revival! The show shocked fans by making JD, played by Zach Braff, the new Chief of Medicine at Sacred Heart, replacing John C. McGinley’s Dr. Cox. This was a well-kept secret! While it seems logical considering the show’s been off the air for over 15 years, some might question why Dr. Cox would choose JD. However, the reasoning behind his decision is actually quite clear.

Dr. Cox became the acting Chief of Medicine in Season 7 of Scrubs and officially took on the role permanently in Season 8. Many viewers thought he would continue leading Sacred Heart Hospital for much longer. While it felt natural for JD to eventually take over from his mentor, it happened surprisingly quickly. However, if Cox was considering retirement around the time JD returned, there was only one other doctor qualified to lead the hospital.

Why Dr. Cox Didn’t Choose Elliot as His Successor at Sacred Heart

I’ve always admired Elliot Reid, played by Sarah Chalke. She and JD actually started at Sacred Heart Hospital at the very beginning, back in the first episode! And honestly, aside from a short time at a different hospital during Season 5, she’s dedicated her entire career to that place. She’s just as committed and knowledgeable as JD is, which is saying something! So, when Dr. Cox was looking for someone to eventually take over as Chief of Medicine, I always wondered why he never considered Elliot. She’s clearly more than capable of handling the job.

Looking at Elliot’s career path explains why she wasn’t offered the Chief of Medicine position. By the end of Scrubs, she’d left Sacred Heart to start her own private practice, though she still worked at the same hospital. It would have been difficult, if not impossible, for her to manage both a private practice and the daily operations of Sacred Heart. Plus, private practice often pays better than a hospital administration role, so Dr. Cox probably didn’t think she’d even be interested in the job.

There’s Another Reason Why Dr. Cox Chose JD Over Elliot

Even setting aside the fact that Elliot would have to give up her own practice to become Chief at Sacred Heart, I really don’t think Dr. Cox ever seriously considered her for the position. Their relationship has always been difficult, bordering on hostile. They constantly clash due to their different approaches to things, and while they can be polite when necessary, that’s usually as far as it goes. Some might argue this shouldn’t be a factor, since Dr. Cox hired Turk as Chief of Surgery despite their disagreements. He recognized Turk’s qualifications and ultimately made the best choice for the hospital, and could have done the same for Elliot.

It’s likely Dr. Cox would have considered Elliot for Chief of Medicine if she hadn’t broken up with JD and filed for divorce. He’s always masked his true feelings for JD, but sees him as a son and is very protective of him. This protectiveness was clear when he sharply criticized Elliot for complaining about JD’s return to Sacred Heart, sarcastically saying that the person ending the relationship is usually the one who has the easiest time. Given this strong disapproval of how Elliot treated JD, and the fact that Dr. Cox hoped his successor would share his values, it’s understandable why the Scrubs revival didn’t choose Elliot as the new Chief of Medicine.
