In The Boys, Homelander is increasingly losing touch with reality as he battles enemies like Billy Butcher, and this is especially evident in Season 5, Episode 3, “Every One of You Sons of B*tches.” After learning Soldier Boy survived, Homelander shifts his focus to obtaining Compound V-One, believing it holds the key to true immortality and ultimate power. He’s determined to get it, no matter the cost. (Please be aware that the following contains spoilers.)
He and his father were already estranged, but things have gotten much worse since Homelander essentially tried to kill him. While soaking in a bath of breast milk (naturally), he learns from Soldier Boy that he won’t stop aging. Though his father offered no support, he receives a vision of his former mentor, Madelyn Stillwell, who appears as an angel and encourages him to become even more destructive.
Madelyn Stillwell’s Return In The Boys Season 5 Sets Up Homelander’s Endgame

It’s understandable why Stillwell seems like an angel to him. He’s always had a tendency to think of himself as superior, and his work on V1 is taking that to an extreme – he genuinely believes it will grant him immortality, essentially making him a god. So, it feels right that an angelic figure is supporting him. However, there’s a strong sense of irony here, because she’s actually pushing him towards something disastrous – anyone with sound judgment would see her as a negative influence.
Homelander has always been mentally unstable, but the recent vision clearly shows he’s losing control even further. However, he seems determined to embrace this downward spiral. Stillwell convinces him he’s destined for greatness, urging him to violently punish those who don’t believe and committing horrific acts – hopefully, it’s a level of depravity even The Boys won’t explore.
As a fan, I was completely floored by that Stillwell cameo! It’s always been clear Homelander was fixated on her, even after he… well, you know. We’ve seen hints of it before, but this was different. It really flipped the script on their dynamic. It wasn’t about her controlling him anymore, it was about him desperately wanting her care, that weird motherly comfort he can’t get from anyone – not even Firecracker. Showrunner Eric Kripke explained to The Wrap that this moment is huge for Homelander’s arc going forward, and honestly, it makes so much sense. It shows just how broken he really is.
He’s deeply dissatisfied with everything, despite his power. This leads to what we see as a total mental breakdown. Stillwell then gives him one last task: to achieve immortality, become like a god, and punish anyone who doesn’t genuinely love him, believing that will finally bring him happiness.
It’s clear he’s struggling with his own distorted thoughts, but he’s now convinced he has a purpose – a divine destiny, as he sees it. This belief gives him a newfound freedom. What used to be a private and embarrassing practice – expressing breastmilk – he now does openly in public, even to the point of displaying it for others. He’s completely unconcerned with what people think because he genuinely believes he’s been chosen to become a god, and an angel has confirmed this is his fate. He’s fully embracing his unconventional path.
This suggests Homelander will become even more powerful, even without needing V1. He’s completely unrestrained, doesn’t listen to anyone, and there might be no one capable of stopping him from doing whatever he wants. We see a hint of this when he nearly kills his son, Ryan, at the end of the episode. If he does get V1, stopping him will become incredibly difficult as Season 5 of The Boys reaches its climax.
