Episode 4 of The Testaments Season 1, titled “Green Tea,” starkly reveals the true terrors of Gilead. While the show hasn’t shied away from the regime’s cruelty, earlier episodes presented it differently than The Handmaid’s Tale. By focusing on teenage girls who grew up within Gilead and enjoy a privileged position compared to Handmaids, the show initially portrayed the evil as more subtle and creeping. However, this episode marks a shift towards the more direct and brutal depiction of Gilead seen in the original series. (Please be aware that this discussion contains spoilers and addresses sensitive topics, including sexual assault.)
The story of “Green Tea” concludes with Agnes getting dressed when she realizes her undershirt is undone – and she didn’t open it herself. She slowly realizes that Dr. Grove, the dentist who previously anesthetized her, must have done it. While not explicitly stated, the scene strongly suggests that Dr. Grove sexually assaulted Agnes while she was unconscious.
The Testaments Season 1, Episode 4 Shows The Horrors Of Gilead
This isn’t the first time Agnes has experienced something like this. In a previous episode, Dr. Grove inappropriately touched her during an exam. The show didn’t focus on the incident, and Agnes seemed to try and ignore it. Growing up under a system that didn’t educate girls about sex or consent – and instead prioritized men – she likely didn’t fully grasp what had happened, but it was enough to make her dread future appointments.
Episode 4 of The Testaments doesn’t show us the event itself, but creates a deeply unsettling feeling. As Agnes falls asleep under the care of the unsettling Dr. Grove, we understand something bad is about to happen to her, and that despite Daisy being there, it’s unavoidable. This tension culminates in the episode’s final reveal, making the outcome undeniably clear.
Watching Agnes break down after realizing something went wrong during her dentist appointment is deeply upsetting, especially because she doesn’t understand what happened. This moment marks a turning point for her character. Earlier in the series, Agnes seemed somewhat accepting of Gilead, content with the comfortable life she’d been promised. Now, the true horrors of Gilead are becoming clear to her, and this realization will likely be the first step towards her questioning everything and potentially rebelling.
It’s also unclear whether Paula knew about Dr. Grove’s behavior, especially since she strongly encouraged Agnes to go to the dentist. The book offers a stronger suggestion – Agnes is convinced Paula knew exactly what would happen. This aligns with Paula’s character, as she clearly dislikes her daughter-in-law and seems generally opposed to Gilead’s system. It hints that everyone knew what Grove was doing, but prioritized protecting him – a powerful figure – over safeguarding the girls.

The episode concludes with Daisy realizing the full horror of Gilead, mirroring June’s journey. Overwhelmed by what she’s discovered and understanding there’s no simple escape, she breaks down but forces herself to find strength.
Unfortunately, there’s no simple solution. As Paula has shown, adults either don’t take these concerns seriously or dismiss them entirely. The current system is set up to benefit people like Dr. Grove, not to help those who are struggling. Agnes has very few people she can confide in, besides Daisy. Aunt Lydia might be an option, as she understands the true nature of Gilead, but building that level of trust with her would be difficult.
Watching Daisy and Agnes, it’s heartbreaking to see how much they’ve lost – their innocence, their safety, everything they took for granted. But that’s Gilead for you, isn’t it? Even in the darkest times, there’s always a spark of hope. And that’s what’s so powerful about The Testaments – even with everything awful happening, we get to see these women fight back, and it’s incredible.
